General Information

This form must be completed at least 4 business days prior to check-in.

Big Bass Lake is a private membership community with a 24-hour safety team.
All persons in the rental group must be listed on this form regardless of age. Persons not listed on this form will be DENIED ENTRY.
In compliance with township regulations:

The maximum number of people per rental group during any rental period is 12, regardless of age. (infants count as renters)
The minimum age of all renters is 25 years old unless they are the child of one of the other renters.
The maximum number of cars permitted in the driveway is 8.
NO parking is permitted on the street, in the yard, or in the garage.

Vehicle Pass:

All vehicles MUST obtain a vehicle pass before going to the rental property and vehicle pass must be prominently displayed at all times.
Drivers are required to produce their vehicle plate number and valid driver’s license upon arrival at one of the two security gates. IDs of all guests may be checked at the gate. More detailed information will be provided prior to your arrival.

It is very important that you communicate arrival instructions and the rules of both the community and Larsen Lake House to all persons in your group prior to your arrival.
The Guest Information flyer, which outlines the primary BBLCA rules, as well as more detailed information regarding the community and Larsen Lake House rules will be provided prior to your arrival.

Pets (including emotional support animals) are NOT permitted unless authorized in advance. Additional fees and rules apply.

Amenity Passes may be purchased at the Clubhouse or Administration Office. Amenity Passes are required to use any facility at Big Bass Lake, including, but not limited to, public beach, clubhouse, pool, tennis/basketball courts, etc. Passes for age seven (7) and above are priced as follows:

Weekday Daily Pass: $8 per day/per person
Weekend or Holiday Daily Pass: $10 per day/per person
Week Pass (non-holiday): $30 per week/per person
Holiday Week Pass: $50 per week/per person

By typing my name below, I hereby acknowledge and agree to comply with all rules, regulations, and policies set forth by the Big Bass Lake Community Association. I further understand and accept that I am fully responsible for the payment of any fines or penalties incurred by myself or any member of my party as a result of violations of said rules, regulations, or policies. I understand that any modifications to this application must be submitted via email to host@larsenlakehouse.com no fewer than four (4) business days prior to the start of the rental period. I further acknowledge that failure to do so may result in fines or administrative fees. I agree to comply with all rules specific to Larsen Lake House (LLH) and understand that I am responsible for ensuring that all members of my group do the same. I further agree to maintain the property in a clean and orderly condition throughout the duration of our stay. Lastly, I accept full financial responsibility for any damage to the rental property or its contents that occurs during our occupancy. I agree to promptly report any such damage to the property owners upon discovery.

Waiver and Release of Liability

Release of Liability, Waiver of Claims, Assumption of Risk, and Indemnity Agreement PLEASE READ CAREFULLY. BY TYPING YOUR NAME BELOW AND SUBMITTING THIS FORM, YOU CHOOSE TO WAIVE CERTAIN LEGAL RIGHTS, INCLUDING THE RIGHT TO SUE. To: Michael Hoagland & Michael Eaton, Owners of Larsen Lake House and West End Pond, Gouldsboro, PA 18424 (“Owner”). I, along with the other members of my rental party and our guests (collectively, “Renters”), wish to swim and/or use the watercraft, life jackets, dock, hot tub, sauna, games, equipment, treadmill, and other amenities (collectively, “Gear”) during our stay at your vacation property. By choosing to use the Gear, we acknowledge and fully understand the following: A. Renters acknowledge that the Owner makes no warranties or guarantees, express or implied, regarding the condition, suitability, or safety of the Gear. Renters accept full responsibility for inspecting all Gear prior to use and for determining its usability and safety for themselves and their guests. Use of the Gear is entirely at the Renters’ own risk, and the Owner shall not be held liable for any injuries, damages, or losses arising from its use by any member of the rental party or their guests. B. There is no lifeguard or safety personnel on duty at any time. Renters are solely responsible for the safe operation of the Gear and for ensuring the safety of all members of their party while the Gear is in use. C. Renters understand that the use of the Gear involves certain risks, including but not limited to: 1. Injuries resulting from malfunction or failure of the Gear; 2. Injuries from tripping, slipping, or falling; 3. Injuries caused by unsupervised or improper use of the Gear; 4. Other injuries that may occur in connection with the use of the Gear. D. Renters acknowledge that the risks listed above are not exhaustive, and that other unknown or unanticipated risks may arise. Renters voluntarily assume all such risks. E. In consideration of being permitted to use the Gear, Renters agree to the terms and conditions outlined below. __________________________________ Waiver, Release, and Indemnification Renters expressly agree to the following: A. TO WAIVE ANY AND ALL CLAIMS that Renters now have or may have in the future against the Owner, including its members, directors, officers, employees, agents, and representatives (collectively, the “Releasees”), arising from or relating to the use of the Gear; B. TO RELEASE THE RELEASEES from any and all liability for loss, damage, injury, expense, or other costs that Renters or their guests may suffer, or that their next of kin may suffer, arising from the use of the Gear, INCLUDING LIABILITY RESULTING FROM THE NEGLIGENCE OF THE RELEASEES; C. TO HOLD HARMLESS AND INDEMNIFY THE RELEASEES against any and all claims, demands, actions, or damages to property or personal injury (including death) suffered by any third party as a result of the Renters’ use of the Gear; D. That I, as the representative of all Renters, am at least eighteen (18) years of age and will ensure that all members of the rental party comply with the rules of the property; E. That this Waiver, Release, and Indemnification shall be binding upon all Renters and their respective heirs, next of kin, executors, administrators, assigns, and any person acting on their behalf or on behalf of their estates; F. That if any provision of this Agreement is found to be invalid or unenforceable, the remaining provisions shall remain in full force and effect. ___________________________________ Acknowledgment: On behalf of all Renters in my rental party, I affirm that I have read and fully understand the terms and conditions set forth in this Waiver and Release of Liability. I confirm that I have consulted with all members of my group and am authorized to sign on their behalf. I acknowledge that by typing my name below and submitting this form, I am voluntarily waiving certain legal rights that may otherwise be available to myself or my party against the Releasees, and I do so knowingly and willingly.
